Case Management
Case managers are nurses. They can provide education about your health care. They can work with the family to identify the needs of the child and family. They also work with the primary care provider (PCP) when medical questions come up. They can make referrals to specialists. They work to promote communication between the provider, family and medical specialists. They can assist you with transportation. They can help you with special equipment and home health requests. They can help make sure your needs are being met. They are here to assist you.
Children who need more medical, Behavioral Health or educational needs than other children their age can get special help if they are in a case management program.
Missouri Care supports pregnant women with a special program to help them stay healthy during their pregnancy and make sure they get the health care services they need. If you are pregnant, tell the plan of your pregnancy right away so you that can benefit from the maternity case management program.
