Welcome to the MO HealthNet Health and Wellness Program that serves 16 counties in northwest Missouri. Our mission is to provide you with the tools and help you need to improve your health and wellness.

About Us

MO HealthNet Health and Wellness Program is a local health plan contracted with the state of Missouri to provide a care management program. It is administered by Missouri Care and partnered with Community Health Plan.

Missouri Care has been working with MO HealthNet participants and providers in the state of Missouri for the past 10 years. Community Health Plan, established in 1995, is a leading health care resource in the northwest region of the state.

Our care management solution will help participants in making healthy, educated lifestyle choices. Participants with chronic conditions will learn how to manage their illness or condition through a variety of tools.

We have care management staff waiting to help you choose a Health Care Home, answer your questions or assist in finding local resources to help you.
